The Social Worker provides high quality social work services for patients on the Generation Unit. The social worker is consistent with the Kansas Behavioral Sciences Regulatory Board of Social Workers as well as with OSHA, HFAP, Federal, and State regulations.  The social worker coordinates and develops the mental health aspect for our patients. 


Clinical and Discharge Responsibilities
1.   Upholds Newton Medical Center vision, mission, and core values.
2.   Follows hospital-wide and departmental policies, including but not limited to general procedures, assigned duties, confidentiality, and observing the Patient’s  Rights.
3.  The Social Worker is responsible for quality (HBIPS) and appropriateness of social work services.     
4.  The Social Worker conducts periodic audits of the quality of social work services furnished as assigned.
5.  Demonstrates effective communication skills with all team members, patients, and families.
6.  Conducts family meeting/education sessions to review patient progress toward goals, discharge planning and/or areas of concern or questions pertaining to community resources, patient needs and other general information within the scope of social work.
7.  Assesses and applies appropriate interventions and documents process and outcomes. Develops and updates  detailed reassessment of the patients for the Generations Unit.
8.  Performs written psychosocial assessment within 72 hours of admission which addresses patient's strengths, limitations and needs on the Generations Unit.
9.  Attends and participate in team meetings and/or ensure appropriate social work representation is provided at team meetings for the Generations unit. Social worker will communicate discharge plans with team members for each unit as anticipated from point of admission and update as needed.
10. Demonstrates an appropriate level of knowledge of geriatric psychopathology and current DSM diagnosis utilized on the Generations Unit.
11. Will submit social work progress notes according to unit standards.
12. Conducts process group sessions daily or as assigned on the Generations Unit.
13. The Social worker assists the patient and families in identifying strengths, stressors and goals.
14. Works toward realistic goal setting and helping patient reach identified goals.
15. Accurately identifies resources needed for appropriate aftercare planning.
16. May perform screens as assigned on the Generation Unit per regulation requirements.

Professionalism, Interpersonal Relationships and Communication

1. Effectively relates with physicians in matters relating to patient care.
2. Expresses dissatisfaction/complaints only to appropriate persons.  Maintains a positive attitude.
3. Accepts changes in assignments or schedules willingly.
4. Serves as a role model for all staff, promoting and displaying professional behavior e.g. Remains busy, reads only professional literature on duty.
5. Provides atmosphere conducive to learning and mental well being.  Accepts constructive criticism as a means of self-improvement.

Organizational Responsibilities

1. Completes annual CPR certification prior to anniversary date.
2. Actively participates in mandatory meetings i.e., unit, education etc. for improvement of patient care or department efficiency.
3. Participates in facility QAPI program.
4. Promotes NMC’s image and maintains a helpful attitude towards employees, physicians, patients, visitors, and the community to enhance NMC’s reputation as a health care center.
5. Assists with Orientation of new personnel.
6. Must adhere to the unit, nursing, and NMC policies and procedures e.g., infection control, safety. 

Job Requirements

 Knowledge and skills:
 Must have communication skills necessary to provide appropriate information to the age of the patients served.
 Education and experience:
 Masters Degree in Social Work from a school, accredited by the Council of Social Work Education preferred.
 Two years post LMSW experience working in mental health or geriatrics. 

 Licensure, registration or certification:
Current license to practice social work in the state of Kansas.  Basic Life Support.
